Affidavit\uff09\uff1a\u516c\u53f8\u5b9d\u85cf\u58f0\u660e\u5df2\u6536\u5230\u81f3\u5c1125%\u7684\u80a1\u672c\u91d1\u3002<br>\u8463\u4e8b\u4f1a\u6210\u5458\u58f0\u660e\uff08Board of Directors&#8217; 2303\uff08\u6ce8\u518c\u8d39\u5355\uff09<br>\u8d2d\u4e70\u56fe\u4e66\u548c\u5370\u82b1\u7a0e\u7968<br>SSS\uff08\u793e\u4f1a\u4fdd\u9669\u4f53\u7cfb\uff09\u3001PhilHealth\uff08\u56fd\u5bb6\u533b\u7597\u4fdd\u9669\u673a\u6784\uff09\u548cPag-IBIG\uff08\u4f4f\u623f\u4e92\u52a9\u57fa\u91d1\uff09\uff1a\u4e3a\u5458\u5de5\u548c\u516c\u53f8\u81ea\u8eab\u6ce8\u518c\u3002<br>\u5e02\u653f\u6216\u5730\u65b9\u653f\u5e9c\uff1a\u83b7\u53d6\u5e02\u653f\u8bb8\u53ef\u8bc1\u6216\u4e1a\u52a1\u8bb8\u53ef\u3002<br>\u5176\u4ed6\u8bb8\u53ef\uff1a\u6839\u636e\u884c\u4e1a\u53ef\u80fd\u4f1a\u9700\u8981\u989d\u5916\u7684\u8bb8\u53ef\u8bc1\u6216\u8ba4\u8bc1\uff08\u5982\u98df\u54c1\u536b\u751f\u8bb8\u53ef\u3001\u73af\u4fdd\u8bb8\u53ef\u7b49\uff09\u3002<\/li>\n\n\n\n<li>\u5176\u4ed6\u8003\u8651<br>\u516c\u53f8\u5370\u7ae0\u548c\u94f6\u884c\u8d26\u6237\uff1a\u5236\u4f5c\u516c\u53f8\u5370\u7ae0\u5e76\u5f00\u8bbe\u516c\u53f8\u94f6\u884c\u8d26\u6237\u3002<br>\u96c7\u4e3b\u8d23\u4efb\uff1a\u5982\u679c\u6709\u96c7\u5458\uff0c\u4e86\u89e3\u5e76\u9075\u5b88\u96c7\u4f63\u6cd5\u89c4\uff0c\u5305\u62ec\u6700\u4f4e\u5de5\u8d44\u3001\u798f\u5229\u7b49\u3002<\/li>\n<\/ol>\n\n\n\n<figure class=\"wp-image-20396\"\/><\/figure>\n\n\n\n<p>\u8d39\u7528\u548c\u65f6\u95f4\uff1a<br>\u8d39\u7528\uff1a\u9664\u4e86SEC\u7684\u6ce8\u518c\u8d39\u7528\uff0c\u8fd8\u6709BIR\u3001\u5730\u65b9\u653f\u5e9c\u548c\u5176\u4ed6\u673a\u6784\u7684\u8d39\u7528\uff0c\u603b\u8d39\u7528\u4f1a\u56e0\u516c\u53f8\u7c7b\u578b\u548c\u89c4\u6a21\u800c\u5f02\u3002<br>\u65f6\u95f4\uff1a\u6574\u4e2a\u8fc7\u7a0b\u53ef\u80fd\u9700\u8981\u51e0\u5468\u5230\u51e0\u4e2a\u6708\u7684\u65f6\u95f4\uff0c\u5305\u62ec\u6587\u4ef6\u51c6\u5907\u3001\u5ba1\u6279\u548c\u5404\u673a\u6784\u7684\u6ce8\u518c\u6d41\u7a0b\u3002<\/p>\n\n\n\n<figure
